Macfarlane was U.S. Nuclear Regulatory Commission chairman from 2012 to 2014. From 2010 to 2012, she was on the Blue Ribbon Commission for America's Nuclear Future. Prior to that, she co-edited the book Uncertainty Underground, about the proposed radioactive waste dump at Yucca Mountain, Nevada, on Western Shoshone Indian land.
